Information that we have

Brewood Pilates will hold the relevant information that is required to take part in the Pilates classes. This will allow the management team to make any necessary adjustments to the class content to support your needs. Also, our governing body will expect us to have their documents completed in full and stored securely.  

All clients will complete and sign an enrolment form that will require the following:

  • Name
  • Address
  • Email address
  • Phone numbers (mobile and landline)
  • Emergency contact number
  • Date of Birth – All ages are welcome
  • Occupation
  • Sex
  • Any Sports / Hobbies
  • Reason for exercise
  • A full medical history that is relevant to the class physical exercise
  • Notification of any pending/ future health treatments 
  • Your signature 

How we get the information and why we have it

All clients will be asked directly to complete the enrolment form so they can attend the classes on the days allocated. With the information, we have a duty to inform clients as to whether a class has been cancelled, any new classes time, standard class times and any other Pilates related information that maybe useful or of interest.

The information regarding our client’s medical status is required because we must ensure that they attend the appropriate class suitable for their physical ability. 

Also, to request payment and/or agree a payment method which you may receive reminders or/and a copy of your invoice/ receipt. Which can be receive by:

  • Text message
  • Email
  • Paper form in person

Do we personal information indirectly?

Brewood Pilates does not receive personal information indirectly, however, should this happen then we would always request for them to contact us directly or request a formal introduction in person.

If you are named as an Emergency contact, we would have a legitimate reason for contacting the named person, however we would request that our client would inform them that we have their details. In addition to having the information we would expect that should there be any changes in details we would need to be informed. (GDPR – Accuracy requirements)

The website has the capability to gather information through Google Analytics, SEO, Blogs, Social Media, and email campaigns which encourages more visitors to our website.

Under the General Data Protection Regulation (GDPR) we have a duty to contact you on the basis that we have a legitimate interest. This will allow us to process your request in line with our business procedure.

You (our clients) have the right to the following:

  • Withdraw your consent
  • Access to your information card
  • Update your information
  • Restriction of some information
  • opt in or out of our marketing campaigns

Your information may be required for the following whereby we will require a Letter of Authority:

  • GP’s
  • Physical therapist 
  • Accountants

  • Insurance

How we store your information

Your information will be stored securely offsite in highly secure lockable cabinet that is fire and flood proof. 

Should you request a copy of the information that we hold on you then it will be provided in paper form within 28 days from the date of your request.

We will keep a section of your information for 8 years, due to HMRC requirements, Governing Body or for any other legal reasons.

Under data protection law, you have rights including:

Your right of access – You have the right to ask us for copies of your personal information. 

Your right to rectification – You have the right to ask us to rectify information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete. 

Your right to erasure – You have the right to ask us to erase your personal information in certain circumstances. 

Your right to restriction of processing – You have the right to ask us to restrict the processing of your information in certain circumstances. 

Your right to object to processing – You have the right to object to the processing of your personal data in certain circumstances.

Your right to data portability – You have the right to ask that we transfer the information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you which is stated on the ICO (Information Commissioner’s Office) website.
